REQUIREMENTS:

• Graduate from ACPE accredited Pharm D Program
• Current full, active, unrestricted license (in ANY state)
• BLS
• Clinical Pharm residency or fellowship accredited by ASHSP or ACCP 
  OR 
• Documentation of appropriate education, training, and /or CME in the practice of Clinical Pharmacy
