Licensed Social Worker (Mental Health)

Loyal Source is looking for a Licensed Social Worker (Mental Health) for an employment opportunity in Durango, Colorado.

Position Overview:

The Licensed Social Worker (Mental Health) will be instrumental in providing comprehensive mental health social work services to Veterans and their families. This position involves conducting biopsychosocial assessments, delivering counseling sessions, and coordinating resources to address various psychosocial and mental health needs.

Qualifications:

• Must have a Master’s degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited school.
• Current, full, active, and unrestricted license in Colorado.
• Must meet VA qualification standards for Social Worker as outlined in applicable VA policies.

Position Responsibilities:

• Conduct comprehensive biopsychosocial assessments to ascertain the needs of Veterans, their families, and caregivers.
• Deliver a full spectrum of clinical social work services, including treatment planning, and individual, family, and group counseling.
• Make referrals to VA and community-based resources, providing case management services as required.
• Implement tailored interventions addressing housing, homelessness, finance, family support, legal, education, employment, disability, mental health, addiction, medical issues, and abuse/neglect.
• Collaborate in treatment planning with interdisciplinary teams and coordinate with other programs as needed.
• Assist with long-term care placement, discharge planning, transitions between levels of care, and arranging in-home services to ensure timely and appropriate care.
