Advanced Practice Registered Nurse (APRN)

Loyal Source is looking for an Advanced Practice Registered Nurse (APRN) for an employment opportunity in Durango, Colorado.

Position Overview:

The Advanced Practice Registered Nurse (APRN) will be responsible for the comprehensive care of both assigned and unassigned patients, including development and implementation of patient care plans, leadership within the care team, and continuous improvement of care processes. This role emphasizes face-to-face and virtual care access, and collaboration with PACT staff to address patient needs.

Requirements:

• Must possess a master’s degree from a program accredited by the National League for Nursing Accreditation Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
• Maintain full and current certification as a Certified Nurse Practitioner (CNP) from the American Nurses Association or another recognized certifying body in Adult Health or Family Practice.
• Authorization for prescriptive authority is required. CNPs must prescribe controlled substances following the Controlled Substances Act and state licensure regulations.
• Must have at least three years of clinical nursing experience, with a minimum of one year as a CNP (three years preferred).
• Experience in outpatient care within Family Medicine or Internal Medicine is preferred.
• Must have a current, full, active, and unrestricted license and registration as a graduate professional nurse in Colorado.

Key Responsibilities:

• Provide healthcare services aligned with licensure and clinical privileges, focusing on clinically indicated care.
• Offer health care services to patients within the Patient Aligned Care Team (PACT), including development of care plans tailored to patient consent.
• Lead the care team by delegating care responsibilities and reviewing clinical data to enhance team processes.
• Ensure patients have same-day access for face-to-face and virtual care during clinic hours.
• Collaborate with PACT staff to create personal health plans incorporating care management and coordination.
