Loyal Source is looking for a Registered Nurse (RN) Care Manager for an employment opportunity in Durango, Colorado.
Position Overview:
The Registered Nurse Care Manager (RNCM) will provide comprehensive nursing care to both enrolled and unassigned patients at the site, enhancing patient safety and quality by collaborating with the Patient Aligned Care Team (PACT) to develop and manage care management plans. The RNCM will engage in multiple modes of communication and care delivery to ensure patient-centered, coordinated care.
Requirements:
- Must be a graduate of a professional nursing school accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
- Must possess a current, full, active, and unrestricted registration as a graduate professional nurse in Colorado.
- Must meet VA qualification standards for Nurses as outlined in VA policies.
Key Responsibilities:
- Provide professional nursing services consistent with licensure and professional standards, fulfilling all aspects of care management.
- Collaborate with PACT staff to develop, oversee, and manage care management plans and care coordination for assigned patients.
- Utilize various communication modes, such as secure messaging, telephone care, clinical video telehealth, and face-to-face visits, for care delivery.
- Identify patient needs and coordinate with discipline-specific team members to enhance care management.
- Assume full accountability for assignments related to care management, care coordination, nursing services, and outcomes of care.
- Enter orders into the VA Electronic Health Record (EHR) for tests according to standardized RN care management protocols or PCP orders.
- Ensure same-day access for face-to-face and virtual care visits using evidence-based practice standards to promote patient engagement and wellness.
- Collaborate with the PACT to improve patient care outcomes and enhance access and continuity of care.
- Use advanced clinical knowledge and critical thinking to mentor and guide staff in implementing interventions that improve patient outcomes.
- Design and provide age and population-specific health promotion and risk reduction strategies.
- Translate evidence-based research into practice to ensure patients benefit from the latest nursing innovations.
- Manage patients in transition between levels of care and serve as an expert resource, implementing skills like motivational interviewing to promote patient self-management.
