Licensed Clinical Social Worker

Loyal Source is looking for a Licensed Clinical Social Worker for an employment opportunity in Durango, Colorado.

Position Overview:

The Licensed Clinical Social Worker will be responsible for providing comprehensive social work services to Veterans and their families. This role includes conducting psychosocial assessments, leading counseling and therapy sessions, and coordinating with community resources to address a wide range of social and clinical needs.

Qualifications:

• Must have a Master’s degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited school.
• Current, full, active, and unrestricted license in Colorado.
• Must meet VA qualification standards for Social Worker as outlined in applicable VA policies.

Position Responsibilities:

• Conduct biopsychosocial assessments to identify the needs of Veterans, their family members, and caregivers.
• Provide a full range of clinical social work services, including treatment planning, individual, family, and group counseling.
• Make referrals to VA and community-based resources and manage casework for Veterans as necessary.
• Provide tailored interventions addressing housing, homelessness, finance, family/caregiver support, home care, legal, education, employment, disability, mental health, addiction, medical issues, and abuse/neglect.
• Actively participate in treatment planning with interprofessional teams.
• Assist with long-term care placement, discharge planning, transitions between care levels, and arranging in-home services to ensure timely and appropriate care.
