Clinical Pharmacy Practitioner

Loyal Source is looking for a Clinical Pharmacy Practitioner for an employment opportunity in Durango, Colorado.

Position Overview:

The Clinical Pharmacy Practitioner (CPP) will provide advanced pharmacy services, focusing on comprehensive medication management and chronic disease state management. This role involves working as an advanced practice provider, delivering direct patient care under a scope of practice or collaborative practice agreement.

Qualifications:

• Must be a licensed pharmacist with a degree from an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy (BS Pharmacy and/or PharmD).
• Requires at least 1 year of pharmacy equivalent experience performing relevant activities.
• Must hold a current, full, active, and unrestricted pharmacy license in Colorado.
• Must meet VA qualification standards for Licensed Pharmacist as specified in VA policies.
• Authorization for a VA scope of practice is required per VHA Handbook 1108.11 Clinical Pharmacy Services.
• Must have post-graduate experience in comprehensive medication management and chronic disease state management, such as a post-graduate residency or equivalent experience under a collaborative practice agreement.

Position Responsibilities:

• Provide pharmacy services to both assigned and unassigned patients, ensuring comprehensive medication management.
• Schedule with bookable appointments and structured grids to optimize patient care.
• Function as an advanced practice provider using a scope of practice or collaborative practice agreement.
• Collaborate with other healthcare providers to ensure effective patient management.
