Loyal Source is looking for a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for an employment opportunity in Durango, Colorado.
Qualifications:
- Must hold a current, full, active, and unrestricted license in Colorado.
- Must meet VA qualification standards for Licensed Practical or Vocational Nurse as outlined in applicable VA policies.
Key Responsibilities:
- Provide covered services to both assigned and unassigned patients at the care site, offering evaluation and care consistent with licensure, certification, and practice elements.
- Collaborate with Patient Aligned Care Team (PACT) staff to develop comprehensive healthcare and care management plans for assigned patient panels.
- Manage clinic workflow to ensure patients are placed in examination rooms promptly and provide direction as they navigate the clinic.
- Perform a variety of specialized clinical support skills and basic patient care services.
- Demonstrate knowledge of medical terminology, interpersonal communication skills, aseptic technique, infection control, and patient confidentiality policies and procedures.
- Assist healthcare providers in delivering patient-centered care effectively and efficiently across all clinical areas.
- Receive training to serve as a backup for telehealth services.
