Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)

Loyal Source is looking for a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a notional employment opportunity in Kingston, New York.

About the Role:

The LPN/LVN will provide essential clinical support and patient care services to veterans at a Community-Based Outpatient Clinic (CBOC). This role supports the Patient Aligned Care Team (PACT) by ensuring efficient clinic operations, delivering direct patient care, and contributing to the development of comprehensive care plans. The LPN/LVN plays a key role in managing clinic workflow and enhancing the patient experience through timely and compassionate care.

Key Responsibilities:

• Provide covered services to assigned and unassigned patients presenting for care.
• Deliver patient care consistent with licensure, certification, and functional statement with elements of practice.
• Collaborate with PACT staff to develop and implement health care and care management plans.
• Manage clinic workflow, including timely placement of patients in examination rooms and guiding them through the clinic environment.
• Perform a variety of specialized clinical support skills and basic patient care services.
• Demonstrate knowledge of medical terminology, aseptic technique, infection control, and patient confidentiality.
• Communicate effectively with patients and staff, supporting a patient-centered care environment.
• Assist healthcare providers in delivering clinical services across all areas of the clinic.
• Receive training and serve as back-up support for telehealth services as needed.

Required Qualifications:

• Current, full, active, and unrestricted license as an LPN or LVN in the state of New York.
• Meets VA qualification standards for Licensed Practical or Vocational Nurses.
• Demonstrated ability to perform clinical support duties in a healthcare setting.
• Strong interpersonal communication skills and knowledge of patient care protocols.

Preferred Experience:

• Experience working in a VA or government healthcare setting.
• Familiarity with outpatient care workflows and electronic health records (e.g., CPRS/VistA).
• Prior exposure to telehealth services or willingness to be trained in telehealth support.
