A Registered Nurse in Disease Management will support daily clinical operations at the 628th Medical Group, Joint Base Charleston. This role involves providing various professional medical and medical support services, significantly impacting patient care and service delivery within the organization.

Key Responsibilities:
- Coordinate patient care in collaboration with healthcare professionals, facilitating optimal outcomes in clinical care, quality, and cost-effectiveness.
- Ensure compliance with standards of care and practice according to established policies, procedures, and guidelines.
- Perform physical exams and health histories.
- Provide health promotions, counseling, and education.
- Administer medications, wound care, and other personalized interventions.
- Direct and supervise care provided by other healthcare professionals.
- Make patient care assignments based on the scope of practice and skill level of assigned personnel.
- Recognize adverse signs and symptoms and react quickly in emergency situations.
- Communicate and collaborate with diverse groups for informing healthcare team plans/actions, and for patient/family education.
- Make referral appointments and arrange specialty care as appropriate.
- Perform nursing services and conduct research to improve practice and patient outcomes.
Required Qualifications:
- Associate’s degree in nursing is required.
- Graduation from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
- A minimum of one-year experience in nursing.
- BLS/CPR (AHA OR ARC) certification required prior to starting the position.
- Current, full, active, and unrestricted license to practice as a Registered Nurse.
Schedule: Monday through Friday, excluding Federal Holidays, between 0730-1630 with a 30 to 60-minute unpaid meal break (not exceeding 40 hours per week).
Benefits Offered: W2 pay package with paid leave, holidays, and eligibility for health insurance options.
Pay: $37.50 per hour.
