Loyal Source Government Services is looking for 10 full-time Community Living Center Registered Nurse at Louis Stokes Cleveland VA Medical Center in Cleveland, OH

Job Summary: Community Living Center Registered Nurse must provide professional medical services and quality care to our Veteran population. Located at the Louis Stokes Cleveland Veterans Affairs Medical Center (LSCVAMC) in Cleveland, Ohio. Community Living Center Registered Nurse are required to deliver high-quality, compassionate, and consistent patient care.

Duties:

  • RNs provide short-term, post-acute skilled nursing and rehabilitation services to patients who require intensive clinical care and therapy following hospitalization, surgery, or acute illness.
  • RNs assigned to work in the CLC will be responsible for patient assessments, medication administration, IV therapy, drawing labs, central and peripheral line IV medications, tube feedings, foley catheters, wound care, trach care, assistance with activities of daily living, interdisciplinary team coordination, admissions, discharges, internal and external transfers, delegating to assistive personnel, electronic medical record documentation, patient transport, and emergency response procedures.
  • All RNs assigned to CLC inpatient under this contract will be required to float between units based on staffing needs.
  • The registered nurse collaborates with an interdisciplinary team to provide comprehensive care to Veterans that are completing rehab with the goal of returning to their prior functional status after an acute medical stay.
  • The registered nurse collaborates with an interdisciplinary team to provide comprehensive care to the Veterans that require more respiratory focused care then what can be supported in the home environment.
  • The registered nurse utilizes the nursing process to provide comprehensive care to Veterans that are admitted for rehab, IV antibiotic management, and simple to complex wound care.
  • The registered nurse provides comprehensive holistic care to veterans that are admitted to the floor due to respite, placement needs, short term rehab or in need of medical care that cannot be safely given at home.

Qualifications:

  • Required to have two years of Community Living Center experience within the last three years.
  • Graduated from a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • A current license from the Ohio Board of Nursing or a current multistate license from a Nurse Licensure Compact (NLC) State.
  • Current certification in CPR by an American Health Association Vendor
  • Required to have a current Basic Life Support certification.
  • Required to have current ACLS certification.
  • Required to have basic typing and PC computer skills.

Additional Information:

  • VA operations are twenty-four hours a day, seven days per week (24/7), including holidays.
  • Shifts are primarily 12-hours, with some units offering 8-hour and 10-hour shifts.
  • RNs shall be available to work on days, nights, evenings, weekends, and Holidays.
